I haven't checked this recently (I wrote a couple workarounds that I
keep up to date for the problem). For a long time, multiple copies of
each plugin showed up in the addin-db directory. The solution for me was
a simple dictionary check to see if a plugin was loaded. I can send you
the patch if you want to include it. I just figured the real problem was
more related to multiple interface references in the code.

Description: 
Exception thrown in LoadDefaultSharedModules line 79 when adding the
DynamicTextureModel. The m_loadedSharedModules dictionary appears to
already be populated (did loaddefaultsharedmodules get called already?).
The current call stack comes from
OpenSim.ApplicationPlugins.LoadRegions.
