Make an HTML page called http_loginform.html in your Opensim/bin 
directory and that will; be served as the content area of the SL 
Viewer if you use the loginuri parameter to your SL Viewer.  We use 
ours to show a full screen image snapshot of our region for example. 
There is an example in the bin directory that may give you some 
ideas, but we don;t use the login form fields in that.
