Additional Informationy 256.0181 in first error message and x 256.0402 in second error message are at (just outside) boundary of a 256x256 regions
Region A is not DefaultRegion, DefaultHGRegion or FallbackRegion
No agent in Region A at time of error
No assets or scripts at locations given in error
No such error reported for other 8 regions on same simulator
No such error reported after 14:33:10
Simulator started at 13:26:03

There must be something that is positioned just outside the region. This could be some prim that got positioned outside or a physical ball or something that rolled to the edge of the region.

This error occurs when BulletSim tries to sanity check the position of an avatar or prim and, when making sure not below the terrain, can't get the height. I might add more information to the error message so the offending prim can be identified.

Another error, this time for a different region:

18:20:11 - [BULLETSIM TERRAIN MANAGER]: GetTerrainHeightAtXY: terrain not found: region=Region B, x=122.4687, y=-0.09976107

This time preceded by (related, not related ?):

Error in red: 18:16:44 - [WATCHDOG]: Timeout detected for thread "Heartbeat-(Region B)". ThreadState=WaitSleepJoin. Last tick was 5413ms ago.
Warning in Yellow: 18:20:12 - [WATCHDOG]: 208511 ms since Watchdog last ran. Interval should be approximately 2500 ms

There is noting at those coordinates in-world so it could be something in the database that has not rezzed but without more information it will take time to find what it is as I will have to find items by using these coordinates.

In each case, delete object outside - Delete all scene objects outside region boundaries returned no objects were found outside region bounds.

Searching Prim table by GroupPosition coordinates matching those in the reported errors resulted in "Not Found".
